All candidates for ASA office are asked to submit the following information no later than February 15th for distribution with the annual ballot.

• Name as it should appear on the ballot
• Present Position
• Former Professional Positions Held (list up to 3)
• Education (list degree earned, university, major, and year)
• Offices Held in ASA (list up to 5 with most recent listed first)
• Offices Held in Other Organizations (list up to 5 with most recent listed first)
• Publications (List up to 5 with most recent listed first)
• Personal Statement (up to 150 words)
